| If your utility bill becomes delinquent, we are required by the Public Service Commission rules to send disconnect notices ten days prior to the contemplated date of disconnection. If there is no response to this notice, a 24 hour disconnect notice will be delivered to your address by the police dept. This will be your final notice. Your service WILL be disconnected if you do not respond to this notice. A written work order will be issued, signed by the mayor, authorizing disconnection. The utility dept. will then pull the meter and leave a notice at the property. All payment negotiations are to be done at the utility office. The utility workers are not authorized to collect or negotiate payments. Utility workers are only responsible for completing the work order, which means disconnection of service and delivery of notice. | |
| Your service WILL be disconnected on or after date shown on notice unless: | |
| 1) | the account is paid in full |
| 2) | arrangements are made to pay by signing a deferred payment agreement. If payment is not made on the agreed date(s) a subsequent notice shall be left on the premises 24 hours prior to disconnection. |
| Immediately contact the utility office: | |
| A. | If you dispute the notice of a delinquent account. |
| B. | If you wish to negotiate a deferred account. |
| C. | If any resident is seriously ill. |
| D. | If there are other extenuating circumstances such as: infants, young children, aged, or handicapped residents, residents on life support systems or equipment, residents who have mental retardation or other developmental or mental disabilities. |
| Service will be continued or restored for 21 days if you submit a statement from a licensed Wisconsin physician or notice from a public health or social service official identifying the serious illness of a resident and the period of time during which disconnection would aggravate the illness. | |
| You may appeal to the staff of the PSC if an agreement cannot be made concerning the reason/s for disconnection or the amount of the utility service bill. Their phone number is 1-800-225-7729. | |
| If electric or water service has been disconnected, and you wish to be reconnected, you must pay: | |
| 1) | your past due account balance in full, |
| 2) | a reconnection charge of: $35/electric($30/water) between 7:30am-3:OOpm OR $60/electric($40/water) after 3:00pm., and |
